Our friendly staff is available to assist you with scheduling your visit, answering questions, verifying insurance information, and helping you prepare for your appointment.

 

At The Family Practice of Arizona, we want your visit to be as smooth and productive as possible. Preparing ahead of time helps our healthcare team provide you with the best possible care.


Before Your Appointment


Please take a few moments before your visit to prepare the following information:

  • Write down any questions or concerns you would like to discuss with your provider.
  • Make a list of all medications you are currently taking, including prescription medications, over-the-counter medications, vitamins, and supplements.
  • Bring information about any recent visits with specialists, urgent care centers, or hospitals.
  • Update us on any changes in your health, medications, allergies, or medical history.
  • Complete any required patient forms before your appointment, if available.


What to Bring to Your Appointment


Please bring the following items with you:

  • Photo identification
  • Insurance card(s)
  • List of current medications
  • Medical records or test results from outside providers, if applicable
  • Any forms provided by our office that need to be completed
  • Co-pays are due at time of service. For your convenience co-pays can be paid by cash, Visa, MasterCard, Discover, American Express

 

Arriving for Your Visit


  • New patients are asked to arrive 20–25 minutes before their scheduled appointment time to allow enough time for registration, completion of paperwork, insurance verification, and review of medical history.
  • Established patients are encouraged to arrive 10–15 minutes before their appointment to allow time for check-in and updating any necessary information.
  • Please notify our office as soon as possible if you are unable to keep your appointment so we can assist you with rescheduling.
  • Arriving late may affect your appointment time and may require rescheduling.


New Patients


If you are a new patient, please be prepared to provide additional information, including:

  • Complete medical history
  • Previous healthcare provider information
  • Medication history
  • Allergy information
  • Family medical history
  • Insurance and contact information


Telemedicine Appointments


For virtual visits, please make sure you have:

  • A reliable internet connection
  • A smartphone, tablet, or computer with camera and microphone access
  • A quiet, private location for your appointment
  • Your medications and health information available for review
